Position title: Forms Designer
Organisation: Ballarat Health Services
Employment Type: Full time, fixed term– 80 hours per fortnight with ADO until December 2022
Pay rate: Commensurate with skills and experience
Ballarat Health Services, proudly part of Grampians Health provides a complex range of surgical, medical, obstetric, psychiatric, palliative care, rehabilitation and residential services. Ballarat is located approximately 75 minutes drive west of Melbourne and offers a wide range of recreational, sporting and social facilities for a population of over 90,000 people. A vibrant, progressive regional city, Ballarat is large enough to retain its own regional flavour but is within easy commuting distance to Melbourne.
We are seeking applications for a Forms Designer to join our team. The main purpose of this position is to develop electronic clinical record forms within BOSSnet Digital Medical Record (DMR). The role will also be responsible for management of paper medical record forms as required.
Clinical Forms Design – electronic and paper
- Design, build, and modify electronic forms within BOSSnet.
- Design and modify clinical paper forms as required.
- Ensure all new and revised forms comply with internal and external standards
- Work in close collaboration with Forms HIM, BOSSnet Business Analysts, regional partners and HIS Operations Manager to coordinate Digital Medical Record (DMR) content.
- Health Information Committee secretariat.
Key selection criteria include:
- Skills in electronic form/web design and build
- Excellent attention to detail, thoroughness and follow through
- Excellent interpersonal skills – verbal and written
- Intermediate to Advanced level skills in Microsoft Word and Publisher
- Qualified Health Information Manager (desirable)
